A Case of Hemorrhagic Fever with Renal Syndrome Resulted in Chronic Renal Failure and Hypertension

Abstract
A 42 year-old-male patient admitted with febrile disease for 5 days with nausea, vomiting and oliguria. Initial peripheral blood cell count revealed markedly left-shifted leucocytosis and thrombocytopenia and the blood chemistry showed azotemia.
We
confirmed HFRS (Hemorrhagic fever with renal syndrome) by clinical findings and four-fold increase of Hanta virus antibody by indirect immunofluorescent technique. After discharge, renal deficit was persisted and hypertension was newlv developed.
Biopsy
findings performed at 15 weeks after discharge showed intersticial fibrosis and mononuclear cell infiltration associated with focal tubular atrophy, which were consistent with interstitial nephritis. The kidney size by initial sonography was
relatively
normal but the sonography at 15weeks after discharge showed lobulated and contracted kidneys. Serum creatinine was persistently elevated, that was 3.26mg/dl at 12 month after discharge and the blood pressure should be controlled with
antihyertensive
prescription. He absolutely had no revious history of hypertension and disease. We concluded that HFRS resulted in chronic renal failure and hypertension in this case.
